Assurance Wireless Application.  Assurance is a government phone provider providing phones and no-cost.  However, you must qualify financially to receive free service. The Assurance Wireless Application form has changed a little.  Today you are now required to provide proof of eligibility, due to abuses in the government phone program.  If you are low-income or enrolled with a federal assistance agency, you must attach proof of financial status with the application.  The application is pretty simple, but you may appreciate a breakdown of the process.

How to Qualify

First of all, you can work and receive a free phone.  Many people may think, if they work, they are not eligible for a government phone.  However, if your income is low, you may qualify for a free government phone.  Eligibility goes by your gross family income, not whether you work or not.  If your total family income is at or below 135% of the federal poverty guidelines, your family can receive help.   Those who work 40 or more hours a week, and still live below the federal poverty standards. Find out if you are eligible.  go to the Assurance Wireless site to sign up for this free service.  You can do many things on the website such as contact them by email, check your eligibility, check the status of your application, do your annual recertification, and troubleshoot any problems you may have with your phone.

Benefits You Receive

If you are eligible for the Lifeline program,  you can receive:

  • Free cell phone
  • 500 free minutes every month
  • Unlimited free texts

You will also receive an additional 150 free voice minutes for the first four months of new service.

  • Purchase additional time in the amounts of 5 or $30 or international calls.
  • Buy a Virgin Mobile Top-Up cards at where phones and phone accessories are sold.
  • And pay with cash credit card or PayPal.

Assurance Wireless Application Questions you must Answer

Request for Personal Information

Enter the State you live in.  Next,  a page to download your application will appear. (on the right) This part of the Assurance Wireless application is self-explanatory; just fill out the requested blank fields: Last Name, First Name, Date of Birth, Last 4 digits of your Social Security Number, Home phone number and if applicable your E-Mail Address, Physical Address.   Don’t forget the apartment or house number. (check box if the address is temporary).

Program-Based Eligibility

Here you will find a list of government programs.  Check the box to the right, indicating any programs you are currently where you are receiving assistance.  You are Required to Provide proof of program participation, and or gross yearly income such as:

  • Photocopy of your Benefits ID Card
  • A letter from a qualifying agency
  • Statement of your benefits for the current or previous year

Income Based Eligibility

This is where you will check the box, with your family size and the amount that closest identifies your gross yearly income.  Provide 3 months of statements such as:

  • Pay stubs
  • SS Benefits
  • Veterans Benefits Statements
  • Retirement or Pension statements
  • Unemployment Benefits statement

Provide One of Each Document

  • State or Federal Income returns
  • Income statement from job
  • Federal letter of General Assistance
  • Divorce papers or child support documentation

How to Check the Status of your Application

Once you have applied after a few days you can check the status of your application. This is much easier than trying to reach someone on the phone or going through a lot of confusion phone prompts. This is how to check your status. Go here.  enter your 6 – 10 number account application identification and your home zip code. Here you can see that date you submitted the application, whether or not your approval letter was sent and when or if your phone has been shipped.

UPDATE –  Assurance is a Lifeline-supported program. The benefits provided are only eligible for those who qualify.  Because for waste or fraud in the program, stronger rules have been put in place.  Anyone willfully providing incorrect information can receive a fine or face imprisonment and barred from the program.  Each household is eligible for one phone and free minutes. These benefits are not transferable to anyone.
