Who Qualifies

If you believe you qualify, check out this site to find what Lifeline providers are servicing your state, go to their website and apply.  You will need to provide proof of your financial status with, recent payroll stubs, letter from social services, or w-2 statement.   If approved, they notify you by email or regular mail.

How to Apply:  To register, go to the Assurance Wireless website and download and application or call their toll-free number to request an application be mailed to you. Your application must be accompanied by proof of income, by submitting, current payment stubs, W-2 forms, or certification from the program you are receiving assistance from.

    I recently sent my info for recertification on Monday and phone is still off I downloaded the application and proof by fax I need to speak to someone

      Precious, they usually contact you a month before your certification date to avoid interruption of service. Contact your provider to find out what is happening.
