To enroll in the Lifeline program, you must prove eligibility by producing documents to verify identity, proof of income, or government assistance. When applying, please attach,
- Statement from the following: Department of Social Services Medicaid, SNAP Benefits, Veteran’s
- Administration, Social Security Benefits.
- Current Federal Tax Return
- Letter from Unemployment/Workman’s Compensation
- A payroll statement or current check stubs from your job
- Notice letter of participation in a qualifying federal or Tribal program.
